Terms and Conditions
Multiple referrals must be made in the same 12 month period. New 12 month period resets referral count. Referees name must be noted on NP paperwork to be eligible. Referral rewards vouchers are awarded to EXISTING patients of record at Albury Family Dental only at time of referral. Vouchers are not awarded to minors 21 years or under. Credit vouchers are valid for 6 months from creation. Expiry date on voucher. All vouchers mailed to referrers address, and it is their responsibility to keep this updated.
